Referral Instructions
To refer a patient to Alberta Retina Consultants, please follow the steps below:
1. Complete the Consultation Form
Please fully complete this mandatory consultation form and fax to our office for each referral.

You are also welcome to include your usual referral summary letter if it provides helpful background.​
2. Appointment Confirmation Process
​Once we receive the referral:

We will contact the patient directly to confirm their appointment date and time.

We will make two attempts to reach the patient.

If we are unable to confirm the appointment after three attempts, the referral will be cancelled.
In that case, your office is welcome to resubmit the referral after confirming the patient’s availability.
